Regional Service Excellence Award Nomination Form

You may nominate yourself, a peer, a team, a department, or an organization that you feel deserves recognition for regional emergency preparedness activities. Nominations are anonymous  but you may leave contact information if you wish to do so. Nominations will be reviewed and recipients selected by the Emergency Preparedness Planning Council (EPPC) Award Committee. Recipient(s) will be announced and awards presented at regularly scheduled EPPC meetings.
